Use of Hydrogen Breath Testing for Diagnosis of Dietary Fructose Malabsorption

چکیده
Fructose is a simple monosaccharide which is found in many plant foods. It is also used as a sweetening additive in the form of fructose corn syrup, a mixture of glucose and fructose. Its consumption is, in general, rising among the population. Setting aside the condition of hereditary fructose intolerance (which is relatively rare), it is possible for dietary intake of fructose to overload the enterocytes. This is consistent with the limited absorption capacity of the enterocyte transporter GLUT5, which is used by fructose to cross the intestinal epithelium [1]. Non-absorbed ffructose produces gastrointestinal symptoms from its metabolism by colonic bacteria, which may be an undiagnosed cause of recurrent abdominal pain. It has also been suggested that fructose malabsorption may underlie the gastrointestinal symptoms seen in patients with irritable bowel syndrome [2].

Fructose malabsorption
Incomplete intestinal absorption of fructose might lead to abdominal complaints such as pain, flatulence and diarrhoea. Whether defect fructose transporters such as GLUT5 or GLUT2 are involved in the pathogenesis of fructose malabsorption is a matter of debate. The hydrogen production by colonic bacteria is used for diagnosis with the hydrogen breath test. However, the appropriate fructose test...
متن کاملBenefits of breath hydrogen testing after lactulose administration in analysing carbohydrate malabsorption.
BACKGROUND AND AIMS Breath hydrogen testing after lactulose administration may yield findings of clinical value, but whether it should be a routine part of breath testing has not been evaluated. We examined the contribution of breath testing after lactulose administration to the conduct and interpretation of breath hydrogen responses after fructose and lactose administration. METHODS Two hund...
متن کاملFructose and lactose intolerance and malabsorption testing: the relationship with symptoms in functional gastrointestinal disorders
BACKGROUND The association of fructose and lactose intolerance and malabsorption with the symptoms of different functional gastrointestinal disorders (FGID) remains unclear. AIM To investigate the prevalence of fructose and lactose intolerance (symptom induction) and malabsorption and their association with clinical gastrointestinal (GI) as well as non-GI symptoms in FGID and the outcome of d...
متن کاملHow to Interpret Hydrogen Breath Tests
Hydrogen breath tests using various substrates like glucose, lactulose, lactose and fructose are being used more and more to diagnose small intestinal bacterial overgrowth (SIBO) and lactose or fructose malabsorption. Though quantitative culture of jejunal aspirate is considered as gold standard for the diagnosis of SIBO, hydrogen breath tests, in spite of their low sensitivity, are popular for...
متن کاملA Case Report of Glucose-Galactose Malabsorption in Iranian Child
Introduction Glucose-Galactose Malabsorption (GGM) is an autosomal recessive and rare disorder of intestinal transport of glucose and sodium-glucose cotransporter type (SGLT1). Case Report Our patient is a 32-day-old boy who was examined for severe diarrhea and acidosis and was treated with GGMdiagnosis. A number of laboratory tests were performed on this patient as well as positive test for r...
